REGISTRATION PAPERWORK REQUIREMENTS

**PLAYER/PARTICIPANT MUST REGISTER ON BOTH SITES- PASCO ATHLETIC LEAGUE AND NPRBUCS.COM**


The paperwork and documentation requirements by Pasco Athletic League are as follows, please upload these documents to your account once registering on www.PascoAthleticLeague.com : 

The following documents are required before your athlete will be cleared to practice:

1. Birth Certificate

2. Proof of Residency (Lease, utility bill, auto registration, etc)

3. Physical from the current year - Must be completed within the year 2024 and on the FHSAA Physical form (found here https://fhsaa.com/documents/2023/3/3/EL2_Form.pdf)

4. Driver License of parent/guardian

5. Current photo of child

6. Online initials and date for Code of Conduct

7. Online initials and date for Concussion Form


You will also need to register separately and pay any registration fees at www.NPRBucs.com
