REGISTRATION PAPERWORK REQUIREMENTS

**AS OF 4/15/24 IMPORTANT PAPERWORK INFORMATION: Player/Participant Contract & Medical Release form will be available for e-signature on June 1st. Please wait to complete any Pop Warner forms until notified by Compliance that they are ready to accept paperwork. **

The paperwork and documentation requirements by Pop Warner are extensive, but this ensures that all teams are in compliance with the rules and policies set forth by Pop Warner National.

The following documents are required before your athlete will be cleared to practice:

1. Original Birth Certificate- to be returned once certified by Pop Warner. Bring to in-person registration, dates TBD.

2. Participant Contract Form (2024 Form) - Available for e-signature 6/1

3. Physical/Medical Release Form (2024 Form) - Available for e-signature 6/1

4. 2023-2024 Final Report Card- Upload to Sports Connect when available at the end of this current school year.

5. $150 Volunteer Deposit Check (if you didn't pay the Volunteer Opt-Out fee) & Volunteer Policy Agreement- Bring to in-person registration, dates TBD.

6. $300 Equipment Deposit Check (turn in at Equipment Distribution, date TBD) & Equipment Deposit Agreement- Bring to in-person registration, dates TBD.

7. Parent Code of Conduct- Bring to in-person registration, dates TBD.

Birth certificates

**DO NOT UPLOAD YOUR BIRTH CERTIFICATE ON SPORTS CONNECT AT THIS TIME. PLEASE BRING TO ONE OF OUR IN-PERSON REGISTRATION DAYS (DATES TBD)**

Pop Warner requires that a member of our governing league (FYD Pop Warner) verify all participant birth certificates. FYD staff must have the original or a certified copy (with raised seal) to look at--photo copies will NOT be allowed. Once they verify the authenticity of the birth certificate, they will stamp a copy (provided by The Bucs) which will be added to the Team Book. After FYD has stamped the copy, the original will be returned to you. Once your athlete's birth certificate is on file, you will not have to provide it again for future seasons.

The parent name listed on the Player Contract MUST EXACTLY MATCH the parent's name on the athlete's birth certificate. If you have legal guardianship of your athlete, you will need to provide a copy of the court document granting guardianship--this has to go in the Team Book and FYD will require it.

Returning Athletes: We will need your original birth certificate as well so we may have it verified/certified by FYD Pop Warner.

HELPFUL TIPS:
- Make sure the participant's name exactly matches their birth certificate.
- Make sure the Name of Parent/Guardian exactly matches their birth certificate or guardianship documentation.
- The Parent/Guardian who's name is on the contract and signs the contract, should be the same person that completes and signs all the rest of the documents.
- Signature of Participant - If the athlete is too young to sign, you can have them draw an X or initial on the line.

report cards

Our Fall 2024 athlete's will need to provide the 2023-2024 Final Report Card when it becomes available this summer.

The copy must be clear and legible (no photographs).

The following information must be shown on the report card:
- School Name
- Participant's Name
- School Year
- Student's Grade Level (e.g. 1st Grade, 4th Grade, etc.)
- All 4 academic quarters or 2 semesters.

Kindergarten students are exempt from the scholastic requirement.
Home schooled? Submit the 2023 Home School Form (waiting on Pop Warner to release 2024 form).

Either bring a copy with you to In-Person Registration (dates TBD) or email it to [email protected].

recent photo

We will need a recent photo of your athlete to upload into Pop Warner National's roster. You don't need to worry about this--we will take a picture at Equipment Distribution, Cheer Uniform Fitting or at a practice you attend.
